West Virginia publishes no per-building quality score: its fee schedule prints only each building’s total rates. Every tier, point and per diem here is SuperLTC’s scoring of CMS’s public long-stay quality measures on the Bureau for Medical Services’ published cut points (State Plan amendment WV-24-0004 and BMS’s “Quality Scoring Metrics” document), read from the CMS release the rule names for each rate date. Dollars share 90% of the $60 million pool on each building’s Medicaid days from its CMS cost report; the pool’s yearly inflation is not published, so it is held at $60 million. Operators are CMS’s chain names; map points are CMS’s geocodes. No patient data appears here.
